We are delighted to have you join us, in enhancing your health and wellness journey. We offer a proven blend of ancient medicine practices with modern advancements, and are devoted to providing comprehensive, compassionate and effective care so that you can become healthy, happy and uplifted.
For your convenience, we have provided a few documents to acquaint you with the office practices and policies of Kimia Wellness, and to answer questions you may have regarding our clinic. The doctor-patient relationship requires both cooperation and mutual trust and we want you to feel comfortable in your communication with us. By working together, we can achieve your health and wellness goals.
Please arrive half an hour early to your first appointment so as to fill in the intake paperwork. Alternatively, you can also download the attachments below, fill them out and bring them with you, so that we can get right to the fun stuff, making you feel amazing!

We’re glad you’re taking your first step towards wellness. Whether you are seeking to heal the body, mind or spirit, we believe these are interconnected, and offer holistic treatments that help all your ailments.
When you come to your first appointment, you will begin by completing the in-take forms, after which you will receive a one-on-one consultation with your clinician regarding your condition and symptoms. The clinician will go over your case and medical history in detail, diagnose the root of the problems you are seeking to treat, and develop a comprehensive and individual treatment plan. You will then enjoy your first therapy treatment with one of our dedicated therapy professionals. Please allow 60 to 90 minutes for your first appointment.
We look forward to supporting you, and helping you reach your optimal physical and emotional health. Our clinic is conveniently located with parking in the back.
Policies
Insurance: We gladly accept United Health Care, Blue Cross, Blue Shield, Aetna and Cigna insurance plans. For all other insurances, we are happy to provide you with a Superbill, which you may submit to your insurance company for private reimbursement.
Payment Policy: Full payment is due at time of service. Accepted forms of payment are cash, check and credit card (Visa and Mastercard).
Cancellation Policy: Please provide a minimum of 24 hours notice when canceling or rescheduling an appointment. In the case of last minute cancellations or a missed appointment, you will be charged a cancellation fee.
Refund Policy: All services, supplements and herbs are non-refundable. We do not offer reimbursements for the full or partial price of any unused services, packages or gift certificates purchased.
